An Advanced Care Plan or Advanced Care Directive is a legal document in which you can outline:
- your instructions about future medical treatments that you can either consent to or refuse; and/or
- your values and preferences for your Medical Treatment Decision Maker (MTDM) to consider when making future decisions on your behalf.
This document works in tandem with your Medical Treatment Decisions Maker Appointment document, as the instructions and preferences outlined are for your MTDM to follow.
Your Advanced Care Plan is used when you no longer have decision-making capacity to make your own decisions.
An Advanced Care Plan is a document that you prepare in conjunction with your Medical Practitioner.
